National Award Nomination Form
If you know someone who has made an outstanding contribution to the photography profession extending past the borders of the state of Kentucky use this form to nominate them.  Nominations open every year in September and close the first day of the Fall Seminar when the National Committee meets to make their decision.

Kentucky Award Nomination Form
If you know someone who has made an outstanding contribution to the photography profession in the state of Kentucky use this form to nominate them.  Nominations open every year in October and close on the first day of December.   The KPPA board will make its decision at their December meeting.

Kentucky Award for Sustaining Members Nomination
Nominate the person or company you've seen give outstanding service to KPPA, its members and the profession of photography.

Director Ballot   This link will be active in December of every year.
Starting in December of every year you may vote to fill any open position on the KPPA board from the list on candidates willing to serve and who have been nominated by the Fall Seminar.  Please nominate any member that you feel will help this association grow and continue to be one of the best of its type in the country.  Voting opens December 1st and closes December 31st of each year.

KFD Merit Claim Form   
Every year you earn merits toward your KFD degree through membership and service.  Merits may be claimed in December for your work in the current calendar year.  Please claim your points by December 31st by using this form.
